How GLP-1 Medications Are Monitored at Elevate Health and Wellness
Written by Kristy Beard, MSN, APRN, FNP-C — Board-Certified Family Nurse Practitioner and Founder of Elevate Health and Wellness, serving patients across the Gulf Coast in Gautier, MS and Foley, AL.
Prescribing Is Only the Beginning
The medical weight loss landscape has been transformed by GLP-1 receptor agonist therapy — but the explosion of telehealth-only services and subscription-based prescribing platforms has revealed a troubling gap in many programs: the prescription is written, the medication is shipped, and the patient is left to navigate the experience largely on their own. At Elevate Health and Wellness, Kristy Beard, MSN, APRN, FNP-C takes a fundamentally different approach. The prescription is the beginning of a monitored clinical relationship — not the end of one.
The Pre-Treatment Evaluation
Before any GLP-1 medication is initiated at Elevate, every patient undergoes a comprehensive pre-treatment evaluation. This includes a thorough health history to identify contraindications — including personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2, which are absolute contraindications to GLP-1 therapy; history of pancreatitis, which requires careful risk-benefit discussion; and significant gastrointestinal conditions that may be worsened by GLP-1-induced gastroparesis. A complete GLP-1 readiness lab panel is ordered including thyroid function, a comprehensive metabolic panel evaluating kidney and liver function, HbA1c and fasting glucose, lipid panel, CBC, and in appropriate patients, amylase and lipase. Vital signs and BMI are documented as a baseline.
Monitoring During the First 90 Days
The first 90 days of GLP-1 therapy are the period of highest variability — when side effects are most likely, dose adjustments are most frequent, and the monitoring relationship is most valuable. At Elevate, patients on GLP-1 therapy have access to direct communication with Kristy's team between appointments for questions about side effects, dose timing, and symptom management. A follow-up appointment is scheduled at 4 to 8 weeks to assess tolerance, review weight progress, evaluate any concerning symptoms, and determine whether dose escalation is appropriate based on both efficacy and tolerability. Patients experiencing significant nausea, vomiting, or abdominal pain receive prompt clinical evaluation to rule out pancreatitis or other GI complications.
Ongoing Lab Monitoring
Repeat lab work at 3 months and 6 months of therapy evaluates metabolic response — including improvements in HbA1c, lipid levels, and metabolic markers — and monitors for potential complications including kidney function changes related to significant weight loss and fluid shifts, thyroid function stability, and CBC for any hematological changes. Patients who are losing weight rapidly are monitored for signs of nutritional deficiency — particularly protein inadequacy and micronutrient depletion — and supplementation is adjusted accordingly. Annual comprehensive lab panels replace the more frequent monitoring once patients are stable on maintenance therapy.
When to Adjust or Discontinue
Not every patient achieves optimal results on their initial GLP-1 medication or dose. At Elevate, plateau in weight loss after adequate dose escalation, significant intolerable side effects, or the emergence of new clinical concerns all prompt reassessment — which may include switching between medications, adding complementary interventions, or in rare cases, discontinuing therapy and exploring alternatives. These decisions are made collaboratively, based on clinical data and patient goals, not on a prescribed protocol that applies uniformly to every patient.
